To provide high-quality, person-centred care and support to individuals in line with their care plans, promoting independence, dignity, choice, and well-being while ensuring their safety at all times. The role involves supporting individuals with daily living activities and contributing to a positive, caring environment. To report to the office team observations in the community. To review care plans.
Key Responsibilities:
- Deliver person-centered care tailored to each individual's needs and preferences
- Assist with personal care, including washing, dressing, toileting, grooming, and oral hygiene
- Support individuals with eating, drinking, meal preparation, and nutrition
- Administer or assist with medication in accordance with company policies and training
- Encourage and promote independence and life skills wherever possible
- Support individuals to access the community, attend appointments, education, employment, or social activities
- Maintain accurate, clear, and timely care of records and documentation
- Monitor and report any changes in an individual's physical or mental health
- Safeguard vulnerable adults and children by recognising and reporting concerns promptly
- Follow infection prevention and control procedures, health and safety legislation, and moving and handling practices
- Communicate effectively with service users, families, colleagues, and healthcare professionals
- Respect confidentiality and comply with data protection requirements
- Provide emotional support, reassurance, and companionship
- Participate in team meetings, supervision sessions, and mandatory training
- Promote equality, diversity, dignity, and respect in all aspects of care
